Box 1-2 contains completed questionnaires completed by various facilities in 1967 and is mostly arranged by county code. Boxes 3 and 4 contain duplicate forms with additional information also arranged by county code; as well questionnaires arranged by county name and miscellaneous facilities that received questionnaires. Box 5 consists primarily of material related to the planning and implementation of the survey.
