Files, 1964-1968, and related material associated with the National Institute of Mental Health's inventory of mental health and intellectual disability facilities in Wisconsin, administered by the Division of Mental Hygiene. Records include questionnaire forms completed by mental health facilities, correspondence between facilities and the National Institute of Mental Health, notes and statistics, and miscellaneous publications.
