Scope and Content Note: Applications for nursing home construction and remodeling, 1982-1998, reviewed under provisions of Chapter 150 of Wisconsin Statutes. The applications involve new construction, renovations, and reallocation of beds from one type of Medicaid facility to another for both private and publicly-owned facilities. The files also contain information about nursing home management including operational and care documents; staffing information; care surveys and statements of deficiency; and financial statements used to support the applications; blueprints showing floor plans, elevations, and safety plans; court and administrative hearing materials; application review reports; correspondence; and final reports. A few files consist only of blueprints. Construction contracts, bidding information, and interim project reports have not been retained. The blueprints have been weeded on the same basis as previous accessions.
