Scope and Content Note

Information included in the registers is the name of child, race, sex, date and place of birth, name and occupation of father, name of mother before marriage, birthplace of father and of mother, names of other children, name of physician or person signing affidavit, residence of that person, date of affidavit, date of registration, and any additional circumstances noted. The affidavits contain similar information; they do not include date of registration nor note names of other children but do note whether any were stillborn. The affidavits also include age of father and mother, occupation of mother, father's residence and mother's residence, whether the birth was a single or multiple birth, and the signature of a notary public. Both registers and affidavits are sometimes date stamped for when “Filed in the State Board of Health.”
