Although the amount of information provided on each envelope varies, the following data is generally completed: name; political subdivision; draft and serial numbers; home address; name, address, and relationship of nearest relative; personal history including date and place of birth; citizenship status; race; marital status; Racine County schools attended; occupation; last employer; date entered service; drafted or enlisted; branch of service; rank on entry; reception center; training camps and schools; date of discharge; place of discharge; remarks; and whether photograph is enclosed or not.
