Abstract:
Founded in 1985 as AIDS Resource Center of Wisconsin, Vivent Health is a national model for providing comprehensive, integrated health and social services to HIV patients. The collection documents the development of Vivent Health from a small provider of limited social services to the largest provider of HIV-health care services in Wisconsin and the most comprehensive AIDS service organization in the United States. It includes administrative records relating to governmental advocacy, annual reports, financial statements, strategic plans, board meeting minutes, and originating documents. It also contains photographs, posters, and audiovisual material related to the fight against HIV/AIDS in Wisconsin.

Biography/History

Early responses to the AIDS crisis in Milwaukee were led by the Brady East STD (BESTD) Clinic. Its leadership quickly recognized that AIDS would be particularly devastating for Milwaukee, an urban center with a large gay male population. In January 1985, the BESTD Clinic Board of Directors formed the Milwaukee AIDS Project (MAP). With seed funding from the City of Milwaukee, MAP provided education and training programs to the community. By mid-year it became apparent that MAP needed a broader base of support to meet the increasingly difficult challenges raised by the AIDS epidemic. The AIDS Resource Center of Wisconsin (ARCW) incorporated in August 1985 to provide a corporate structure that enabled AIDS services to expand in Milwaukee and throughout Wisconsin. The BESTD Clinic transferred MAP to ARCW in February 1986 where it became the operating HIV service entity of ARCW.

Over the next decade, ARCW expanded HIV services to include comprehensive support services for people living with HIV, extensive prevention services for the diversity of people at risk for HIV infection, and expansion of HIV services throughout the state to include ARCW facilities in Appleton, Eau Claire, Green Bay, Kenosha, La Crosse, Madison, Milwaukee, Superior, and Wausau. In February 2015, ARCW and AIDS Network (Madison) merged in order to strengthen Wisconsin's response to AIDS. Throughout its history, ARCW has been a staunch advocate for increased funding and enlightened AIDS policies.

The rapid expansion of ARCW's widely renowned HIV medical home continued with the organization's rebranding as Vivent Health on January 1, 2020 and the acquisition of AIDS Services of Austin on April 15, 2020. This followed the acquisition of St. Louis Effort for AIDS on September 1, 2018 and Rocky Mountain CARES on February 1, 2017. These changes further established Vivent Health's national leadership in the fight against AIDS providing unfettered access to care, achieving the finest quality outcomes for patients and achieving financial strength to continue to expand to meet the needs of people with HIV and at risk of HIV.

Advocacy

From the earliest days of the AIDS epidemic, Vivent Health believed it was essential to include government advocacy as a major part of its mission. The government response to AIDS was initially slow and minimal which required strong, effective advocacy to increase funding and ensure enlightened AIDS policies by government at the local, state, and federal levels.

ARCW focused its advocacy efforts on securing and increasing government funding to help build a comprehensive system of HIV prevention, care and treatment services throughout Wisconsin. ARCW's lobbying began with the State Legislature, Governor, and the State Department of Health Services, and soon extended to the City of Milwaukee, Milwaukee County, and other county governments.

Federal funding to fight AIDS appeared in 1990 with the passage of legislation known as the Ryan White CARE Act. However, funding made available by the legislation was not fairly distributed throughout the country. In 1994 ARCW leaders fought successfully to alter the funding formulas to be more fair to areas of the country that were not AIDS epicenters, but still had significant numbers of people with AIDS.


Prevention

Beginning in 1986, ARCW provided AIDS education to the gay community. It conducted massive prevention outreach campaigns with condom distributions in gay bars and the unique Bag Boyz program encouraging self-pride, safer sex, and HIV testing. ARCW also took its prevention campaign to public sex environments and Internet chat rooms where sexual liaisons occurred.

In the 1990s a second wave of the AIDS epidemic affected racial and ethnic minorities with a disproportionately high rate of HIV infection. ARCW responded with a street outreach program bringing AIDS prevention and harm reduction counseling to minority drug users, women, and youth. Lifepoint needle exchange was launched to help prevent HIV infection and overdose deaths among drug users. Powerful prevention marketing campaigns supplemented ARCW prevention programs.


Services

Throughout its history Vivent Health has worked to provide its clients with comprehensive social and health services. ARCW established its legal services program in 1989 to protect HIV patients from discrimination, health care provider refusal to treat, and denial of financial benefits. Vivent Health lawyers also provide estate-planning services and representation in landlord/tenant disputes and creditor/debtor issues. The Vivent Health housing program began in 1987 and continues to provide comprehensive housing services including rent assistance, transitional residential living, and family residential living.

On World AIDS Day, December 1, 1996 ARCW opened its Dental Clinic, which provides full preventive and restorative dental care. ARCW received a donation of dental equipment from the John Doyne Milwaukee County Hospital and employed its dentist to lead the new clinic. The Vivent Health Medical Clinic provides expert HIV treatment along with primary care to meet the needs of its patients. Over time, the Medical Clinic expanded from its inception in Milwaukee to Green Bay and Kenosha. In 2003 ARCW opened its Mental Health Clinic to provide counseling and psychotherapy for HIV patients, many of whom struggled with depression, post-traumatic stress, addictions, and other mental health issues. Ultimately, the clinic was renamed the ARCW Behavioral Health and Wellness Clinic with added services of psychiatric evaluations, addiction counseling and treatment and wellness programs. The Behavioral Health and Wellness Clinic currently operates in most ARCW locations statewide. The ARCW Pharmacy opened in 2011 to assure that all HIV patients have access to life-saving medications. The Pharmacy provides the most advanced HIV medications along with counseling to help patients manage and adhere to their medications. The Dental Clinic, Medical Clinic, Behavioral Health and Wellness Clinic, and Pharmacy are key components of the Vivent Health Medical Center, which has been designated as an HIV Medical Home by the State Department of Health Services and the federal government.


Scope and Content Note

The collection documents Vivent Health's fight against HIV/AIDS from the early years of the epidemic to the present day. It includes material reflecting ARCW's advocacy efforts, governance, services, and fundraising activities.

Much of the collection deals with ARCW's lobbying activities that led to significantly increased AIDS funding and ensured enlightened policies by government at the local, state, and federal levels. The records shed light on the critical role played by ARCW to designate state funding for HIV/AIDS patients since the 1989-1991 biennial budget, to secure a more equitable allocation of federal funds under the reauthorization of the Ryan White Comprehensive AIDS Resources Emergency Act in 1996, and to expand the federal/state Medicaid program in 1997 to include low-income, newly diagnosed individuals with HIV.

The records also document more controversial topics such ARCW's efforts to attract funding for its Lifepoint clean needle exchange program in Milwaukee. Supporters of the idea stressed its success in reducing high risk behavior among intravenous drug users, while detractors felt that such a program would encourage continued drug use. ARCW's efforts were successful and the Lifepoint program continues to be an important component of its community outreach programs.

In addition to Vivent Health's lobbying efforts, other strengths of the collection include: annual reports from 1989 to 2013, minutes of board meetings from 1985 to 2013, and financial statements from 1988 and 2013. The collection also contains excellent visual and audiovisual materials including videos, posters and photographs.

The AIDS Resource Center of Wisconsin donated records in May 2014 (accession 2013-048), September 2014 (2014-024), March 2015 (2014-038), April 2015 (2014-040 and 2014-043), and June 2015 (2014-045); July 2015 Doug Nelson donated a commemorative hypodermic needle and Mike Gifford donated a bottle of drinking water from the cryptosporidium period. Mike Gifford donated a 2018 medallion in September 2018 (accession 2018-010).

Tim Cary and Greg Comly processed this collection in the fall of 2014 through spring 2015. Tim Cary added the items donated in July 2015 to the collection in March 2016. Scene Savers digitized the audiovisual materials in January 2015. Audio preservation master files were created as 96 kHz/24-bit resolution 2 channel .wav files; access copies as 44 kHz/320 kbps bit rate 2 channel .mp3 files. Video preservation master files were created as uncompressed 720x480 (3:2) .avi files. Video access copies were created as compressed 640x480 (4:3) .mp4 files.

Biography/History
1989-1991 State Budget

ARCW proposed and lobbied for the Wisconsin AIDS Initiative, a proposal that for the first time would provide state funding to support HIV/AIDS patients through a statewide centralized case management system called Life Care Services. ARCW won bipartisan support for the proposal.


1991-1993 State Budget

ARCW won support for a $150,000 grant to the Wisconsin AIDS Research Consortium for HIV clinical drug trial research, and for an increase in Life Care Services funding to $1.8 million. To increase the effectiveness of AIDS advocacy, ARCW formed the Wisconsin AIDS Response Network (WARN) comprised of all AIDS Service Organizations in the state.


1993-1995 State Budget

ARCW worked with the Secretary of the Department of Health Services and the Governor to establish new funding in the state budget for early medical intervention services for HIV patients. $680,000 was approved for AIDS Service Organizations to provide physical health assessments, mental health screenings and laboratory testing. This was the first health care-related funding from the state for AIDS Service Organizations. ARCW lobbied for and won an increase of $420,000 in Life Care Services funding. In total, AIDS funding in this state budget increased by $1.1 million and the Life Care Services biennial funding rose to a total of $2.4 million, double the amount initially set for the program in the 1989-1991 state budget.


1995-1997 State Budget

In the 1995-1997 biennial state budget, ARCW lobbied successfully to avoid cuts to AIDS funding and with strong bi-partisan support won a surprising unanimous vote on the Legislature’s Joint Finance Committee to increase Life Care Services funding by $223,000 or 7%.


1997-1999 State Budget

In 1997, ARCW expanded the WARN system to involve citizen advocates from throughout the state. It created AIDS Action Wisconsin and urged concerned citizens to join in advocating for responsive AIDS policies. These advocates were very helpful to ARCW as it lobbied for and won AIDS funding increases in the 1997-1999 biennial budget.


1999-2001 State Budget

In 1999, ARCW held the first AIDS lobby day at the State Capitol with 123 AIDS Action Wisconsin advocates participating. Their advocacy helped ARCW win support for a $300,000 increase in state prevention funding and a $200,000 increase in Life Care Services funding in the 1999-2001 biennial budget.

Biography/History
1994 City of Milwaukee Budget

ARCW initiated its advocacy for City of Milwaukee AIDS funding in 1993 with a $190,000 AIDS funding proposal entitled the Milwaukee AIDS Initiative. ARCW recruited five other agencies providing AIDS services in Milwaukee to join a coalition with ARCW to secure and receive City funding through the Milwaukee AIDS Initiative. HIV services to be funded by the initiative included HIV education and prevention outreach, testing, and case management, food services, housing services, and legal services. Despite initial opposition from Mayor John Norquist, the proposal was eventually funded.


1995 City of Milwaukee Budget

In the 1995 city budget, ARCW advocacy was required when the Personnel and Finance Committee of the Common Council cut $69,200 in historic ARCW AIDS prevention funding from the budget. Following an intensive lobby effort with Council members, ARCW won a major victory to restore the proposed cut and also to retain the Milwaukee AIDS Initiative.


1997 City of Milwaukee Budget

In the 1997 city budget ARCW lobbied for a new funding initiative called the Milwaukee Partnership to Stop AIDS which would bring the City of Milwaukee into partnership with Milwaukee County, the State of Wisconsin, and private sector donors in a comprehensive prevention strategy to reduce HIV infection among the Milwaukee’s 6,000 injection drug users. In total, it was a $1 million program with the City of Milwaukee contributing new funding of $100,000. The Common Council and the Mayor eventually supported an additional $50,000 in AIDS funding for the Milwaukee AIDS Initiative, bringing its total funding to $240,000.


Cryptosporidium Crisis

In spring 1993 the City of Milwaukee experienced a severe contamination of its drinking water with cryptosporidium causing extreme gastrointestinal and diarrheal illness for 400,000 residents. During the crisis, 92 AIDS patients died.

ARCW provided its clients with clean water, and advised them not to drink Milwaukee tap water and to monitor their health in collaboration with Milwaukee area physicians. ARCW also worked with Milwaukee’s City Health Department, Common Council, and Mayor to secure their acknowledgement of the life-threatening nature of cryptosporidiosis for AIDS patients and their cooperation in offering specific recommendations for avoiding and managing cryptosporidiosis among immune compromised individuals.

Biography/History

ARCW extended its advocacy to Milwaukee County in 1994 which resulted in $50,000 of funding for AIDS prevention in the 1995 budget.

The 1997 budget included funding for the Stop Teen AIDS program that focused on reducing HIV infection among high risk youth. During this budget cycle, ARCW also took steps to reduce HIV transmission among gay and bisexual men and reduce public sex activity in Milwaukee County parks.

ARCW advocated for funding to support its Lifepoint clean needle exchange program in the 1999 County Budget. Despite the support of the County Executive and the Finance and Audit Committee of the County Board, the full County Board narrowly voted to remove the $100,000 funding from the budget.

The following year ARCW broadened its Milwaukee County budget request by integrating the needle exchange program with a new HIV & Hepatitis C Prevention & Drug Treatment Harm Reduction initiative. ARCW affirmed that county funding would not be used to purchase needles, but would be used to fund an increase in ARCW prevention and needle exchange staff. With this assurance, the County Executive and the County Board provided $100,000 of funding for the expanded harm reduction prevention program for injection drug users.

Biography/History

ARCW advocacy with the federal government began with participation on the Board of Directors of the national AIDS Action Council whose mission is to bring AIDS public policy advocacy to Congress and the Executive branch. From 1992 to 1996, ARCW President and CEO Doug Nelson was a member of the board of directors of the AIDS Action Council. ARCW COO Mike Gifford served on the board from 1997 to 2002. This participation engaged ARCW in federal government advocacy with significant influence in the development of the national AIDS policy agenda and its implementation by the federal government.

Congress enacted the Ryan White CARE Act in 1990 to improve the availability of care for low-income, uninsured, and under-insured people with HIV. The funding formulas in the legislation awarded significantly more AIDS funding to a few large cities hardest hit by the AIDS crisis—San Francisco, New York, Los Angeles, and others—and less funding to those with fewer AIDS cases.

When the legislation was up for reauthorization in 1994, ARCW examined the funding formulas and discovered that some states received up to $4,000 more per AIDS case than Wisconsin. As a member of the AIDS Action Council, ARCW urged a change in the funding formulas to achieve more equitable Ryan White funding per AIDS case throughout the country. Large AIDS service organizations (ASOs) from high-funded states, however, were in control of the Council and consistently opposed ARCW-recommended changes.

In its quest for equitable AIDS funding, ARCW organized the Campaign for Fairness, a national coalition of ASOs in 47 states that supported equitable funding. Representing the Campaign, ARCW leaders Doug Nelson and Mike Gifford made the case for equitable funding to Congress and the Clinton Administration. Two years of advocacy resulted in a Congressional victory for more equitable funding formulas. When President Clinton signed the Ryan White CARE Act in 1996, the new equitable formulas allocated millions more in AIDS funding for HIV services throughout the nation.

Biography/History: ARCW lobbied the federal Health Care Financing Administration to expand the Medicaid program to include low-income, newly diagnosed individuals with HIV disease. With costly new HIV treatments patients needed early medical intervention to avoid progression to AIDS, to remain healthy and to avoid intensive and costly AIDS treatments. By changing the Medicaid eligibility policy to eliminate the disability requirement and to expand coverage to all low income HIV patients, Medicaid would help to assure HIV patient access to essential care thus reducing progression to AIDS and treatments that would be costly for Medicaid.

Abstract: This is a Milwaukee Public Television special production that focuses on the challenges of living with AIDS as discussed by four HIV patients: Tracey Whitmore, Dale Lyn Brewster, Maurice Jenkins, and David Kierstyn. Biography/History: AIDS Walk Wisconsin is organized by Vivent Health, is held in Milwaukee and is the largest AIDS fundraiser in Wisconsin. It is an annual event, established in 1990, at which thousands of walkers raise pledges that support AIDS prevention, care, and treatment services in Wisconsin. From the beginning there have been well known personalities such as public officials, sports stars and actors as honorary chairs selected each year to help promote the Walk. In the early years a breakfast was held with a program featuring the honorary chairs and corporate sponsors of the Walk. Wisconsin AIDS Walk was the original name of the event which was changed to AIDS Walk Wisconsin along with the addition of a run for those preferring to run rather than walk the route.

Biography/History: The Make A Promise Dinner and Auction is produced is an annual gala event held in Milwaukee that includes an auction, dinner and program at which community leadership awards are presented to individuals and organizations from the community that have made significant contributions to the fight against AIDS. The event originated in 1987 when a small group of gay men decided to host a dinner to raise funds to support ARCW services. At that first dinner they “made a promise” to continue having a fundraising dinner every year until a cure for AIDS is found.

This series documents the operation and development of Vivent Health. The 1985 folder reflects the organizational development phase of ARCW. The minutes of January 1985 are from the Board of Directors of the Brady East STD Clinic that considered the establishment of the Milwaukee AIDS Project. Minutes that follow that meeting are from meetings of the Milwaukee AIDS Project.

The first minutes of the ARCW Board of Directors date from April 1986. Included in these minutes is the historic memorandum explaining the origins of ARCW, its articles of incorporation, and its initial bylaws. Beginning in 1991 the minutes included the board meeting agendas and minutes of standing committees and the Board of Directors. Ultimately the minutes became part of comprehensive Board of Directors meeting packets that included all minutes of the board and standing committees, annual budget proposals, program reports, strategic plan performance, quality assessment reports, fundraising and special event reports and copies of print media coverage of ARCW activities.

Financial statements show increases in government grant revenue and private sector funding, and the impact of the pharmacy on Vivent Health's financial position.

The first financial audit of ARCW was conducted in 1988 and reflects a nine-month fiscal period from April 1, 1988 to December 31, 1988 due to a change in the ARCW fiscal year from a March to a December fiscal year end. Similarly, the 1999 ARCW audited financial statements reflects an eight-month fiscal period from January 1, 1999 to August 31, 1999, due to a change in the ARCW fiscal year from a December to an August fiscal year end. The 1988 and 1989 fiscal period audits are combined and included in the 1989 statement. Similarly the 1990 and 1991 fiscal period audits are combined and included in the 1991 statement.

Biography/History: Special event fundraisers have been an historic Vivent Health strategy for raising private sector donations. They are high profile and high energy events that provide donors with the opportunity to contribute funds to Vivent Health and to gather in solidarity with fellow donors gaining inspiration for continued support of Vivent Health. Annual fundraisers include the Make A Promise Gala, which began in 1986, and AIDS Walk Wisconsin, which began in 1990. These two events draw thousands of Vivent Health supporters and raise hundreds of thousands of dollars every year.

Series: 13. State Designation of ARCW as an AIDS Service Organization, 1997-1998
Biography/History: In June 1997 the Wisconsin Department of Health and Family Services issued a Request for Proposals (RFP) for state designation of agencies as AIDS Service Organizations (ASO). State designation as an ASO was highly important for ARCW as it provided millions of dollars in state prevention, life care services, early intervention and federal Ryan White Care Act funding that the state distributed. The RFP allowed applicants from five distinct public health regions to compete for ASO designation. Consistent with its evolution throughout the state, ARCW applied for ASO designation in the southeast, northeast, western, and northern regions of Wisconsin. It received state ASO designation in those four regions and for the statewide AIDS/HIV Information/Referral Hotline and Resource Center.
Scope and Content Note: This series contains the applications for each region and the statewide hotline/resource center. Inside each binder in the front left pocket are the review scores by the Department of Health and Family Services. Also in this series are the official RFP by the Department of Health and Family Services and the revised ARCW work plans submitted to the Department in January 1998 based on feedback and suggestions of the Department.

Biography/History

In the 1980s, very few United Way agencies accepted AIDS Service Organizations as members and therefore very little United Way funds were designated for AIDS services. Many AIDS organizations applied for funding from their local United Way agencies and were denied.

In Wisconsin, the United Way of Greater Milwaukee was the first United Way to fund AIDS services, and it did so by inviting ARCW to become a member agency in 1990. As a United Way member agency, ARCW was guaranteed funding for its services every year. Having gained United Way membership in Milwaukee, ARCW applied for United Way funding in many areas of Wisconsin where ARCW provided significant services, including prevention education and outreach programs, food pantry services, the dental clinic, the mental health clinic, and patient case management. Additionally, the United Way of Greater Milwaukee was the first United Way in the country to fund clean needle exchange programs.
