Abstract:
Forms submitted monthly by community mental health clinics, containing monthly summary data and individual patient data. The bulk of this accession consists of mental health clinic patient reports, 1968-1975. There are three types of forms, including an Admission Section (form MH-509.1), Evaluation Section (form MH-509.2), and Separation Section (form MH-509.3). The forms are roughly arranged by type of form, clinic number (found on every form), and then by date.
