Abstract:

Log noting for each admission: date last admitted, hospital case number, number that year, number of male or female that year, name, age, marital status, county of residence, by whom committed, type of admission (voluntary or not), birthplace, occupation, address of guardian, brief diagnosis, number of times in hospital, and religion. For discharges: similar information, plus type of discharge, to whom discharged, and condition when discharged. Statistical summaries are included at the end of each month.
