The interview transcriptions and data in box 16 contain personally identifiable
information and are restricted under the Privacy Rule of the Health Insurance
Portability and Accountability Act of 1996 (HIPAA). However, it may be possible
to view redacted copies of the transcripts for use as de-identified examples.

There are no access restrictions on the remaining materials, and the collection
is open to all members of the public in accordance with state law.
The researcher assumes full responsibility for conforming with the laws of libel,
privacy, and copyright which may be involved in the use of this collection
(Wisconsin Statutes 19.21-19.39).
The Institute for Urban Health Partnerships transferred Accession 2013-043 in
March 2014, Accession 2014-009 in July 2014, Accession 2015-012 in September
2015, and Accession 2016-007 in August 2016. Additional materials were added
from a College of Nursing accession (2016-004), which was received in July 2016.
Additional materials were added from a College of Nursing accession (2021-051),
which was received in September 2019.
Phyllis Reske and Brad Houston processed the collection in Fall 2015 to Spring
2016. Ashlie Biddle minimally processed Accession 2016-007 in September 2016
(Brad Houston supervised). Claire Dinkelman added materials from accession
2016-004 to the collection in summer 2017 (Christel Maass supervised). Phyllis
Reske minimally processed accession 2013-043 in June 2018 (Christel Maass
supervised). Brendan Dooley minimally processed accession 2021-051 in March
