Scope and Content Note: Files, 1994-2001, concerning several programs by which long term care was provided to Wisconsin's elderly and disabled, and a pilot project developed to include dental coverage in these plans. Included with the general files on local providers such as Community Care for the Elderly (Milwaukee) and Elderly Care of Dane County are files on the Wisconsin Partnership which provided care to a larger population. The local providers are documented by contracts, financial reports, numerous quality assessments, and information on rates. The Partnership is also documented by files on early program planning. One carton documents a dental care pilot project developed in conjunction with the Wisconsin Dental Association.
