results; notification of sentence data; sentencing portion of court transcript; notice of
parole hearing; parole planning information; pre-parole investigation; parole application
information; Parole Board decision and comments; FBI rap sheets; master file card; program
review recommendations and decisions; chronological history recording; request for release;
order for transfer; notice of case status changes; probation/parole violation warrants;
revocation order and warrant; teletypes; and related materials and correspondence.
The contents of each juvenile case file include, but are not necessarily limited to, the
following documents: picture, face sheets; investigation requests; admission investigations;
social worker reports; department orders; court orders and determinations; reception center
staffing recommendations; request for juvenile pre-parole; pre-parole investigations;
juvenile offender review program progress review reports; program review summaries;
quarterly progress reports; educational summaries; master file card; chronological history
recording; order for transfer; notice of case status changes; revocation findings of fact
and conclusions of law and order; Social Security benefit information; teletypes; and
related materials and correspondence.
The 1967-1977 files, which were weeded prior to microfilming, typically include a summary
face sheet, a chronological history of the case, medical and psychiatric examination and
treatment records, as well as administrative records relating to supervision and discharge.
The original files exist for a small number of cases which were not microfilmed. They range
from T129,300 to T253,749. References to these files are interfiled in the alphabetical
